Malang, November 20, 2025. An addendum to the cooperation agreement has been drafted between the Faculty of Medicine and Health Sciences (FKIK) of the State Islamic University (UIN) Maulana Malik Ibrahim Malang with Karsa Husada Batu Regional General Hospital and Dr. Radjiman Wediodiningrat Lawang Mental Hospital regarding medical education programs in the hospital network (affiliation). From UIN Maulana Malik Ibrahim Malang, the agreement was signed by the Dean of the Faculty of Medicine, Prof.. Dr. dr. Yuyun Yueniwati PW, M.Kes, Sp.Rad(K), Director of Karsa Husada Regional Hospital, dr. Muhammad Rizal, MM, M.Kes, and the President Director of Dr. Radjiman Mental Hospital, dr. Yuniar, Sp.KJ, MMRS. These three parties played the main role in updating and perfecting the previous cooperation provisions.

This addendum was prepared as a form of adjustment to the Cooperation Agreement for the Implementation of Clinical Educators signed on March 18, 2025. The adjustment was necessary considering the new provisions regarding the accreditation of teaching hospitals, including the decree of the President Director of Dr. Radjiman Mental Hospital Number HK.02.03/D.XXXVII/11328A/2025 concerning the Accreditation Team for Teaching Hospitals and the update of the accreditation form which is the basis for the implementation of clinical education at the hospital. Through this addendum, the parties agreed to changes to Article 16 concerning Technical Provisions for Implementation. In the updated provisions, it is explained that all activities in implementing the cooperation must be guided by the technical instructions that are jointly prepared and ratified. In addition, Karsa Husada Batu Regional Hospital and Dr. Radjiman Mental Hospital also assigned official representatives to become members of the Education Coordination Committee and Education Coordination Team as part of the coordination mechanism for the implementation of clinical education.

The parties affirm that the provisions of the Master Agreement remain in effect as long as they are not amended through this addendum. They also allow for further adjustments should new needs arise in the implementation of medical education. This addendum is declared valid and effective as of November 20, 2025, and is an integral part of the main cooperation agreement. Thus, this signing demonstrates the parties' commitment to maintaining the continuity of the collaboration, ensuring it continues to run smoothly and meets their respective needs.

The addendum document was signed in good faith by all parties and prepared in triplicate, each having the same legal force. With the enactment of this addendum, the Faculty of Medicine and Medicine of UIN Maulana Malik Ibrahim Malang, along with Karsa Husada Regional Hospital and Dr. Radjiman Wediodiningrat Mental Hospital, hope to strengthen synergy in the implementation of medical education and ensure the continuity of quality clinical education services within the hospital environment.
